Failure Analysis of FMEA, presented by Dr Bluvband and Dr. Grabov at the Annual Reliability and Maintainability Symposium
The paper presents a profound failure analysis of the conventional FMEA (Failure Modes and Effects Analysis) procedure and shows that it is characterized by rather poor definitions of some steps, high incertainty, and even failures and pitfalls in decision making, throughout the procedure. The paper suggests authors' proven solutions for the possible failures and pitfalls at every FMEA step.
